Job Summary


The Care Coordinator is responsible for ensuring continuity of care within a designated geographical area. You will coordinate the daily care operations, ensuring that each service user receives a personalized programme of personal care and household management based on their Care Plan. This role requires you to manage holiday, sickness, and emergency call assignments, support senior care workers and care workers, and ensure that the delivery of care upholds the highest standards of dignity, privacy, and independence. You will often be the first point of contact during out-of-hours situations, making critical care decisions and providing on-call support.

Key Responsibilities



Call Management and Rota Coordination:

Receive and manage incoming calls from Care Assistants within your designated area. Oversee current weekly rotas, ensuring holiday, sickness, and emergency calls are promptly reallocated. Maintain continuity of care for service users throughout these operational changes. Part time Deliverance of care duties Alternative weekends

Escalation and Crisis Management:

Escalate crisis issues to the appropriate parties (e.g., service user family, Quality Assurance Manager, Registered Manager, or Directors). Act as the first point of contact for Senior Care Workers and Care Workers during out-of-hours incidents, making safe and timely care decisions.

Emergency and On-Call Support:

Provide cover for colleagues during emergencies to ensure uninterrupted care. Participate in the out-of-hours on-call rota, providing emergency support in person if necessary..

Documentation and Communication:

Record all incoming calls daily in an activity log, ensuring accurate handovers and detailed shift-end communications. Document any changes or areas of concern in the database, as well as recording extra calls for the Finance team in the weekly diary. Attend review meetings as allocated, recording notes and updating the database with any follow-up actions.

Liaison and Reporting:

Liaise with social workers, Mental Health Nurses, GPs, service user relatives, and other relevant healthcare professionals. Record and report concern to the appropriate authorities while maintaining meticulous documentation.

Customer Care and Company Image:

Ensure an excellent level of customer care and consistently promote a positive image of the company to service users and their families. Keep updated with current care standards, regulations, and company policies to ensure full compliance.

Team Support and Additional Duties:

Assist with rota planning and management as needed. Undertake any other special tasks assigned by the Registered Manager to support the smooth delivery of care services.

Essential Skills and Experience



Industry Knowledge:

Proven working knowledge and experience in the domiciliary care industry, including a solid understanding of relevant regulations and legislation.

Organisational Expertise:

Strong organisational skills, with a track record of effective staff rota management for Care Assistants.

Communication:

Excellent written and verbal communication skills, essential for clear liaison among team members and external parties.

IT Proficiency:

Good Microsoft Word, Excel, and PowerPoint, along with other relevant software tools for documentation and communication.

Qualification:

NVQ Level 3 in Health and Social Care or equivalent certification or working towards it.
