Job Description

We are recruiting a Community Development Worker to support Smokefree Liverpool's prevention and health inequalities work. This role plays a key part in engaging priority communities, developing trusted local partnerships, and increasing access to stop smoking support for people most affected by tobacco related harm. A community facing role with direct impact on reducing health inequalities Opportunity to work closely with grassroots organisations, voluntary sector partners, and community champions Contributes to prevention, early intervention, and increased referrals into Smokefree services
Main duties of the job
Flexible role with a varied working week and strong public health focus Supportive multidisciplinary team with NHS and local authority links Monday to Friday core working hours Some evening and occasional weekend work to support community outreach and events Flexible working including community based and home working, aligned to service need This role is ideal for someone passionate about community engagement, partnership working, and improving public health outcomes across Liverpool.

Essential

  • Level 3 or working towards an equivalent qualification in a relevant subject area or demonstrable equivalent experience
  • Demonstrable commitment to professional and personal development
  • Willingness to complete the NCSCT training.
Desirable
  • Training completed in motivational coaching/interviewing or equivalent experience
Knowledge
Essential
  • An understanding of the role of voluntary and community sector organisations and the contributions they make to the health and wellbeing of individuals
  • Knowledge of the personalised care approach
  • Knowledge of local communities, (including resources and assets) VCSE and community services in the locality as well as a commitment to work collaboratively with all local agencies
  • Knowledge of factors influencing behaviour and behaviour change approaches
  • An understanding of health and social care infrastructure and how services are provided to the local population
  • An understanding of the factors that can impact on an individual's health and wellbeing
  • Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies
  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
  • Have knowledge of one of the wider determinants of health (for example Smokefree) and commitment to continually develop further knowledge, skills and expertise within this area
  • An understanding of community development and asset based approaches, building on existing community and personal assets

  • Experience and ability to identify risk and assess/manage risk when working with individuals
  • Experience of building relationships and partnerships
  • Experience of working collaboratively with other health care and non-health care professionals to meet patient needs
