Location: Camborne, Cornwall (Hybrid: office / community / home)

Contract: Fixed term until January 2027 (funding confirmed until this date; possible extension)

Hours: 30 hours per week

Salary: 32,485 per annum FTE (pro rata for part-time)

Transformation CPR delivers community-centred services in Cornwall, working closely with Volunteer Cornwall and local health partners to reduce inequalities and improve lives in the area's most disadvantaged communities.

We're proud to run the Community Health and Wellbeing Worker (CHWW) programme, which provides personalised, non medical support to help residents live healthier, more connected lives.

The role



As CHW Manager, you'll lead, support and develop a dedicated team of Community Health and Wellbeing Workers. You'll oversee the day to day management of the programme, ensure safeguarding and compliance standards are upheld, and work with local partners to deliver meaningful, measurable outcomes for local households.

Key responsibilities



Lead and manage the CHWW team, providing supervision, guidance and performance support.

Develop partnerships across voluntary, health and local authority services, ensuring joined up delivery and continuous improvement.

Oversee programme delivery, including safe and effective outreach, information management, GDPR compliance and accurate reporting.

Act as Safeguarding Lead, ensuring robust risk management, safeguarding procedures and staff training.

Monitor budgets and systems, working with the Finance Officer to maintain sound financial and data governance.

Promote the programme through community engagement, local events and partnership activities.

Contribute to sustainability, policy development and the strategic direction of the organisation.

Engage with residents through visits, calls, outreach and door-knocking to build trust and identify needs.

Person specification



Essential



Knowledge of: Health inequalities and possible barriers, safeguarding legislation and practice, GDPR, and community health principles.

Skills: Strong leadership and team management; excellent interpersonal and written communication; high emotional intelligence and resilience; confident IT skills; ability to think strategically and deliver operationally.

Experience: Managing or supervising staff; community or casework experience; building relationships across health and voluntary sectors; overseeing compliance or safeguarding.

Attributes: Inclusive and compassionate leadership; integrity, confidentiality and professionalism; proactive and adaptable approach.

Qualifications: GCSE (or equivalent) in Maths and English;

Desirable



Knowledge of NHS and wider health systems.

Degree or management qualification.

Financial oversight or policy development experience.
