This is an excellent opportunity for an enthusiastic Community Prevention Officer to join our Systems, Performance and Communities team.
The Role:
We are looking for a passionate and community-focused Community Prevention Officer to join our Systems, Performance and Communities team. Reporting to the Community Prevention Manager, you will play a key role in connecting residents with local resources to enhance their independence and wellbeing.
Your role will involve: Acting as a central point of contact for referrals from health and social care.
Developing personalised community support plans for residents.
Collaborating with local groups and services to match residents with appropriate support.
Supporting community capacity building and integration across sectors.
Monitoring outcomes and contributing to service improvement.
What we are looking for:
Experience working directly with communities and in partnership with stakeholders.
Knowledge of local community resources and the voluntary sector.
Strong communication and interpersonal skills.
Excellent organisational and time management abilities.
Confidence using IT systems and managing referrals.
