Job Overview
We need a Community Project Worker to improve access to health services for the most deprived communities in Wakefield, including residents of Wakefield City Centre, Portobello, Belle Vue and Agbrigg.
The Project Worker will engage with residents through various community projects and services, some of which will be in partnership with other organisations.
With the ability to communicate well to groups and individuals, the Project Worker will encourage and support residents to access local health services for chronic conditions, severe mental illness and hypertension.
Responsibilities
will include organising and delivering community outreach, workshops and training, volunteer coordination, resource development and partnership building.
Measures and accountability
will include collecting data and feedback on an ongoing basis, case studies and regular reporting.
The successful applicant will be educated to degree level or have equivalent experience of working at a similar level in health or social care. We are seeking someone with knowledge or experience in project management, continuous improvement approaches and stakeholder engagement skills.
