We are seeking compassionate and dedicated Community Support Workers to provide high-quality care and assistance to individuals in their own homes, during leisure activities and out in their local community. The Community Support Worker will play a vital role in promoting independence, safety, and well-being for clients with diverse needs, including individuals with learning disabilities and/or autism, individuals with mental health difficulties and individuals with physical disabilities. This position offers an opportunity to make a meaningful difference in people's lives through personalised support and compassionate caregiving.
Duties
Assist clients with Activities of Daily Living (ADLs).
Support medication administration and adherence according to care plans
Prepare nutritious meals and assist with meal planning in accordance with dietary requirements
Support client with mobility using equipment
Maintain a clean and safe environment through regular cleaning as applicable
Implement individualised care plans tailored to each client's needs.
Engage clients in social activities to promote mental stimulation and emotional well-being
Document observations, care provided, and any changes in condition accurately and promptly
Collaborate with healthcare professionals, families, and team members to ensure comprehensive care delivery
Experience
Candidates should have prior experience in home & community care, assisted living, nursing homes, or long-term care environments. Experience working with individuals with developmental disabilities, physical disabilities or autism is highly desirable.
All applicants should demonstrate patience, empathy, excellent communication skills, and the ability to work effectively both independently and as part of a team. Lifting capability may be required for certain tasks involving resident mobility assistance. This role is essential for providing compassionate support that enhances the quality of life for those in our care while fostering a respectful and supportive environment for both residents and staff.
You must hold a UK driving licence and have access to a reliable car.

Pay: 12.25 per hour
