Community Support Worker

Job Description

As a Community Support Worker, you will deliver compassionate, person?centred care and practical support to residents living in our schemes and in the local community across the Liverpool City Region. You will work with service users to implement individual support plans that promote independence, wellbeing and social inclusion while maintaining safeguarding and health & safety standards.

You will be based from our Liverpool office but will cover the Liverpool City Region.

Due to the areas this role covers, a car driver with a vehicle and business insurance is essential.



Key responsibilities



Provide high-quality, empathetic support tailored to each resident's agreed plan.

Support daily living skills (cooking, budgeting, personal care prompt only and medication prompting according to policy).

Encourage service users to participate in community activities, employment, education, and social opportunities.

Build positive relationships with residents, families, and partner agencies (health, social care, housing, and voluntary organisations).

Maintain accurate, timely records (care notes, incident logs, reporting) in line with company procedures and confidentiality requirements.

Support tenancy sustainment and domestic management of properties (reporting repairs, promoting communal standards).

Observe and act on safeguarding concerns; follow procedures for referrals and multi?agency working.

Attend team meetings, supervision, and training; contribute to continuous improvement and person?centred planning.

Administer first aid or medication where trained and authorised; accompany residents to appointments as required.

Person specification



Essential



Empathy, patience, and commitment to person?centred support.

Experience supporting adults with care needs (paid or voluntary) -- Drug and Alcohol, supported living, residential, community care or related settings.

Good communication and interpersonal skills, able to build trust and rapport.

Basic IT skills for record keeping and communication.

Ability to work both independently and as part of a team across dispersed accommodation.

Understanding of safeguarding, confidentiality, and dignity in care.

Willingness to undertake relevant training and DBS check (or existing DBS).

NVQ Level 2 Qualification in health and social care or equivalent.

Desirable



NVQ Level 3 Qualification in health and social care or equivalent.

Experience supporting individuals with learning disabilities, mental health needs or complex behaviors.

Medication administration, first aid or risk assessment training.

Knowledge of local services and community resources within the Liverpool City Region.

Shift Pattern



We are looking to cover the hours 08:00 - 22:00, 7 Days a week, average 42 hours a week on a two week rotating shift pattern.

3 Days on week 1 and 4 Days on week 2; this gives employees every other weekend off and a consistent rotation of work days.

Job Type: Full-time

Pay: From 28,151.00 per year
