As a Community Support Worker for Immediate Social Care Limited, you will provide floating support and enablement services to adults aged 18+ with disabilities, learning disabilities, and/or autism with lower-level needs in Croydon. Your role is crucial in helping service users maintain independence, develop life skills, and integrate into the community.

Key Responsibilities
Client Support and Enablement
Deliver person-centred support to service users in their own homes or supported accommodation and at drop-in centres
Assist clients in developing independent living skills, including home maintenance and budgeting.
Provide support in accessing community resources, healthcare services, and social activities.
Conduct regular welfare checks to ensure clients' health and wellbeing.
Housing and Tenancy Support
Help clients reduce the risk of homelessness and prevent tenancy breakdown.
Assist with moving house and addressing tenancy issues for those transitioning to more independent living.
Guide clients in reporting repairs, dealing with antisocial behaviour, and communicating with landlords.
Financial Management Assistance
Support clients in managing finances, including budgeting, setting up direct debits, and accessing relevant benefits.
Provide training on preventing financial abuse in the community and online.
Skills Development and Community Integration
Facilitate travel training to enhance clients' safety and independence in the community.
Support access to volunteering, training, and employment opportunities.
Assist clients in developing connections with local community groups and services.
Health and Wellbeing
Support clients in accessing healthcare services, including GP appointments and medication management.
Implement trauma-informed practices in your support approach.
Report any safeguarding concerns following organizational procedures.
Welfare checks to support with health and wellbeing
Service Coordination and Planning
Collaborate with other community groups to promote joint working and support client independence.
Participate in assessments and reviews to determine appropriate support levels for each client.
Plan and facilitate the gradual reduction of support as clients become more independent.
Person-Centred Support Planning
Utilize a range of person-centred support planning tools to identify clients' aspirations and needs.
Ensure that information and guidance for clients is accessible and relevant to their needs, enabling them to make informed choices.
Member Engagement and Development
Assist clients in finding meaningful ways to fill their time, including work, volunteering, and learning & leisure opportunities.
Encourage mutual support amongst clients to develop their skills, talents, and relationships.
Help clients understand their rights, responsibilities, and obligations as citizens.
Promote clients Safeguarding, maintain Professional Conduct and report all safeguarding concerns to management.
Take appropriate action to ensure that clients are safeguarded against abuse and report any concerns.
Act in a professional manner and communicate effectively with all stakeholders.
Be an effective team-member, cooperating with colleagues and volunteers in the best interests of clients.
Compliance and Cultural Awareness
Comply with relevant legislation and adhere to policies and procedures.
Be culturally aware and promote equality and diversity in all aspects of the role.

Travel required throughout the Croydon borough
