Job Description

We are looking for a compassionate and motivated Support Worker to join our dynamic Community Mental Health Team. You'll be working alongside nurses, social workers, psychologists, and psychiatrists to support adults living with mental health conditions in their recovery journey. As a Community Mental Health Support Worker, you will play a vital role in supporting individuals experiencing mental health challenges within their local communities. Working as part of a multidisciplinary team, you will help service users achieve their recovery goals, maintain independence, and improve their overall wellbeing.



Provide practical and emotional support to service users in their homes and community settings.



Assist service users to develop their daily living skills, including budgeting, cooking, and accessing local services.



Support care planning and recovery-focused interventions alongside other members of the multidisciplinary team.



Encourage engagement in social, educational, and vocational activities.



Monitor service user mental state and escalate concerns as appropriate .



Promote independence, dignity, and choice in all aspects of care.



Work within a multidisciplinary community mental health team, contributing to team discussions.

Key responsibilities:

Direct Support & Engagement



Provide practical and emotional support to service users in their homes and community environments.



Assist individuals in developing daily living skills such as budgeting, meal preparation, and household management.



Encourage participation in social, educational, and vocational activities to reduce isolation and promote inclusion.



Recovery Planning & Monitoring



Contribute to care planning and implement recovery-focused interventions in collaboration with the multidisciplinary team.



Monitor service users' mental health and wellbeing, promptly escalating concerns to clinical staff.



Support service users in setting and achieving personal recovery goals.



Advocacy & Empowerment



Promote independence, dignity, and choice in all aspects of care.

Advocate for service users' needs and preferences within the team and wider community.
