South Shropshire Mental Health Service are looking to recruit two Full Time Support Workers on a Fixed Term Basis until May 2027 for our Assertive Engagement Pilot Project, supporting people with Severe and Enduring Mental Illness.



The team covers across the South Shropshire area, you will be either based in Ludlow or Bridgnorth but cover is expected across the area access to a car for work purposes is essential.



The main aim of the post is to support and offer meaningful interventions to a small group of complex service users that struggle to engage with traditional models of services under the direction of our qualified nurse team



Under the direction of the key worker you will deliver interventions with an allocated group of service users with severe and enduring mental health needs. The interventions will be focussed on assisting the person to identify and achieve their agreed goals of Recovery and help a person explore their barriers to receiving care and looking at creative ways to overcome these barriers. These will include interventions related to engaging in meaningful occupation, accessing employment, assessing and



promoting physical health, social inclusion, activities of daily living and mental health promotion.



Establish and maintain relationships with service users, relative/carers and colleagues



Deliver agreed therapeutic interventions, in accordance with the care plan, to assist the service user to achieve their recovery goals including; improving activities of daily living, social inclusion, meaningful occupation, accessing employment, mental health promotion and physical health promotion.



Undertake physical health assessments including; weight, height, blood pressure.



Responsible for contributing towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.



Regularly support service-users to access a range of activities and services to maintain their independence and promote their recovery.



At all times ensure that own actions support the equality, diversity, rights and responsibilities of individuals and promote independence.



Regularly reports and effectively communicates service users health status and care aims in care records and verbal reports to care coordinators and other members of the multi-disciplinary team



Have an awareness of community resources appropriate for the service user to access.



Involvement in the review of risk, communicating any changes to the Team Nurses and Assistant Psychologist.



You will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews of the service user's care and effectiveness of the interventions provided
