Job Summary:


The main purpose of this rewarding and important role is to provide a supportive service to vulnerable people with mental health.

Our Community Support team assists in maintaining a supportive and homely service that all team members contribute to for the benefit of our service users.

The Community Support Worker improves the lives of vulnerable individuals by facilitating change and providing them with support in the service users' own home or the wider community. As one of SHARP's valuable team members the Community Support Worker offers support in all aspects of a person's life, assisting with housekeeping duties, accessing social events, attending appointments, and helping them to achieve their daily and long-term goals.

Main Tasks:



Regular daytime work covering a rota, with daytime attendance at staff meetings and training. Work within a team providing regular contact and individualised support for Service Users Carry out the role in a way that offers opportunity of experiences and promotes equality for the people we support. Promote the well-being and general health of the people we support and help create surroundings in which they feel secure. Help to maintain a hygienic and healthy home environment. This may include carrying out some household and domestic duties. Provide practical support and advice to service users in all aspects of their lives including their safety and security within the home. Assist service users to arrange servicing and/or repair/replacement of appliances, furnishings, fixtures and fittings, and assist with arranging utility supplies. Encourage residents or homeowners to adhere to their obligations and responsibilities. Encourage positive relationships with neighbours and assist with settling neighbour disputes if they occur. Promote the building and maintaining of positive relationships with peers and family and encourage and accompany service users to recreational and leisure activities. Providing advice and support with life-skills as appropriate encouraging the service user to become an independent and productive member of the community Assist with budgeting, planning weekly meals and accompanying service users to do their shopping. Accompany service users to appointments and meetings in relation to their support needs. Assist in effective communication between other members of staff, attending meetings and training as required from time to time Keep accurate records including electronic notes, efficiently and in a timely manner, maintaining accuracy and confidentiality in all aspect of support and record keeping Report incidents to the appropriate service(s), including emergency services, taking and recording their advice. And where necessary, contact the out of hours on call staff for assistance or advice Keep up to date with SHARP's policies and procedures and ensure these are adhered to Any other duties commensurate with the post Must have a full clean UK driving license and access to vehicle for work purposes.
Recording and reporting incidents are an important aspect of the role; SHARP often deals with complex cases including sometimes serious safeguarding incidents, it is essential that all team members work as part of the team so that any incident is dealt with appropriately and that if the need arises, we can evidence the actions that we have taken.

No job description can include every task that is required to fulfil a role. It is expected that team members will undertake tasks and duties commensurate with the post, accepting changes and improvements as they occur.

Successful applicants will initially be required to pay for the cost of a DBS application, This will then be reimbursed once they have successfully completed their 6-month probationary period.
