Base hub: Lanivet Parish Community Centre (employer), working across Lanivet, Bodmin, Lostwithiel, Par & Fowey
Travel across this area required. Four days across hubs + one day admin/flex.
Purpose of the Role
The Hub Buddy will support four community hubs across Lanivet, Bodmin, Lostwithiel, Fowey and Par (the Bosvena and Three Harbours network area) to strengthen participation, widen engagement, and improve connections between local people and the activities, groups and services available to them.
The role is practical, relational and outward-facing. You will help people find their way into local opportunities; support hubs to manage referrals; build links with professionals such as Social Prescribers; and gather evidence to show what difference the hubs are making.
You won't be providing personal care or regulated support. Your focus is on connection, encouragement and helping each hub extend its reach.
Key Responsibilities
1. Engagement & Participation
Encourage people to take part in hub activities, especially those who may be isolated, anxious, or new to the area.
Follow up one-to-one with individuals referred by professionals (e.g. Social Prescribers, Wellbeing Workers, PCN teams).
Support people to access the most suitable group, activity or service for their needs.
Help identify barriers to participation and feed back solutions.
2. Hub Support & Delivery
Each hub will set its own weekly tasks, but may include:
Helping increase attendance at activities.
Supporting room-hire enquiries and promoting available spaces.
Spotting opportunities for new groups or sessions based on local demand.
Being an extra pair of hands to help with set-up, welcoming people, and general coordination.
Gathering community information and mapping what's available locally.
3. Referrals & Partnerships
Build strong, practical relationships with Social Prescribers, PCN staff, Adult Social Care, health and wellbeing workers, and other professionals.
Make sure referrals to hubs are followed up quickly and appropriately.
Share clear information with partners about what each hub offers.
4. Data, Reporting & Evidence
Keep light-touch records of attendance, referrals, follow-ups and outcomes.
Support hubs to collect the evidence needed for funders (including Gatisman Trust and Stronger Together programmes).
Provide monthly summaries to the Hub Network Coordinator.
Notice patterns, gaps and opportunities and report them back to the hub leads.
5. Communications & Information
Provide up-to-date information for community newsletters, social media, hub noticeboards and local directories.
Help ensure each hub's information is clear, accessible and consistent.
Share stories and insights (with consent) to help show the impact of the hubs.
6. General Duties
Attend Hub Network meetings and contribute to shared planning.
Maintain confidentiality and uphold safeguarding responsibilities.
Manage your own diary and travel between hubs.
Any other reasonable duties that support the aims of the Hubs Network.
Person Specification
Essential
Able to build trust and rapport with a wide range of people.
Confident working independently and managing your own workload.
Practical problem-solver who is comfortable being hands-on in community settings.
Good communication skills, written and verbal.
Able to use basic digital tools (email, shared folders, simple reporting templates).
Calm, reliable and comfortable meeting people in their homes or out in the community.
Willing to travel between hubs across the network area.
Desirable
Experience in community work, health and wellbeing, outreach, or similar roles.
Understanding of local services and challenges around isolation, long-term conditions or low confidence.
Experience working with Social Prescribers, PCN teams or similar health networks.
How the Week Will Work
4 days visiting hubs
- as needed across Lanivet, Bodmin, Lostwithiel, Par & Fowey).
1 flexible/admin day
- follow-ups, phone calls, data entry, partner meetings, reporting.
Success Measures (Year One)
These will be refined by the network, but likely to include:
Increased attendance at hub activities.
Clearer referral pathways and quicker follow-up times.
A simple evidence base showing what's working and what needs improvement.
More people accessing community-based support instead of relying solely on statutory services.
Stronger relationships with Social Prescribers and health teams.
Identification of opportunities for new groups, activities or partnerships.
