Job Description

This is a role for a Registered Nurse Learning Disability/ Mental Health or Registered Nurse with relevant LD/MH experience who is requiring a role that will enable you to practice your clinical skills whilst developing your management knowledge.



As a senior leader within our NHS Continuing Healthcare (CHC) team, the role of the Complex Case Manager is pivotal in ensuring that clinical assessments and healthcare management are executed with the highest standards of professionalism and care.



This role requires a combination of strong leadership, clinical expertise, and a proactive approach to healthcare delivery, ensuring patient care is personalised, effective, and seamlessly integrated with broader healthcare objectives.



Lead a team of Nurse Assessors to provide clinical expertise and guidance



Ensure the CHC team works effectively to maximize individual skills and performance



Ensure that patient care is effective, safe, and delivers agreed outcomes



Support the recruitment process for the CHC service



Provide managerial and clinical supervision/support for team members



Support peer review



Complete ICB mandatory training and annual personal development plan and any required CPD



Ensure team members CPD is up to date and mandatory training completed



Participate in and take responsibility for ensuring that those staff under your management are aware of and fulfil their responsibilities under all appropriate legislation and guidance such as the Mental Capacity Act, The Human Rights Act, and Safeguarding



Ensure that clinical staff are aware of any updates to relevant guidance and protocols in line with policy, case law and clinical research relevant to the case load and clinical discipline



Manage team workload and ensure completion in timely manner.



Complete quality assurance, and verification of all assessments and reviews.



Provide professional support, expertise, and input, where required to support the CHC Appeals team



Ensure compliance with ICB policies and procedures.

Lead a skilled team of CHC Nurse Assessors in providing clinical evaluations and guidance, ensuring the NHS Continuing Healthcare framework is applied meticulously to determine patient eligibility and care standards.



Optimisation of Team Performance:

Actively manage and enhance the performance of the CHC team by nurturing individual strengths and fostering a collaborative environment to ensure high-quality patient outcomes.



Safety and Effectiveness in Patient Care:

Uphold the highest standards of patient care ensuring all processes are safe, effective, and directly contribute to positive patient outcomes.



Recruitment and Team Development:

Participate in the recruitment process for new CHC team members, ensuring candidates meet the high standards required. Provide ongoing 1:1 supervision and support, facilitating professional growth and development. Support the inclusive induction process for new staff ensuring positive learning experiences.



Education and Compliance:

Ensure all team members are up-to-date with mandatory training and personal development plans. Lead educational initiatives that keep the team informed of the latest clinical practices and compliance requirements.
