We seek dynamic nurses or paramedics with amazing interpersonal skills to support the Senior Clinical Practitioners in the county wide Urgent Community Response team. This involves working to get people out of hospital or preventing hospital admission by playing a major part in their journey, setting goals and support planning with Therapists, Doctors and support workers as an integrated approach for patients in their own home environments. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments and assessments in the patient's own home environment.
The Urgent Community Response Team operates as a Virtual Ward across the county, managing patients with sub-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.
In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patients' needs as and when required. The shifts for the service are between 07.00am to 01.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns.

Applicants must hold a valid UK driving licence, have access to a vehicle for work purposes at all times on duty and be confident to drive in all driving conditions for this community post. Applicants will be flexible and adaptable, being able to respond to need at short notice.
