Assist in the development and implementation of integrated care pathways, includingappropriate service user assessments/referrals and models of care. Participate in case supervision to monitor standards of service user care anddocumentation. Participate in regular clinical supervision as per Trust policy Ensure the environment is conducive to promote service users health, safety and wellbeing, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs. Ensure record keeping is in accordance with Professional and Trust standards Ensure the safe management of medicines and adhere to safe practice and principles ofadministration in accordance with the NMC code of professional conduct and TrustMedicines Code.
Work in a professional and collaborative manner with the Multi-disciplinary team. Participate and promote positive intra-agency working. Participate in the referral process of potential new referrals, including the assessment ofsuch individuals in accordance with the agreed care pathway and the completion of quality reports where appropriate Participate and assist in setting standards of service user care based on sound researchfindings Where indicated and following completion of the relevant training the practitioner will act as a non-medical prescriber, at all times working within their sphere of clinicalcompetence.
