To provide support to service users who attend the Bridge Centre NWL in line with the organisation's aim to provide a high quality, reliable support service tailored to suit each of our service users individual support plans, and to promote independence, choice and autonomy.
Duties and Responsibilities:
To work collaboratively with the team leader in the planning, development, delivery of tasks/activities for service users and to differentiate tasks/activities according to the needs of service users.
To support service users on all activities when out in the community.
To be fully involved in the planning and development, of independent social and interpersonal skills and to undertake activities to support the personal, social and emotional needs of the service users.
To assist in the devising of service users individual targets and their monitoring and review.
To assist & be flexible in trips with service users.
To develop positive relationships with service users and staff to assist service users progress.
To monitor and record service users progress on a regular basis and report as required i.e. care plans, risk assessments, incident/accident forms.
To assist in the specific medical/care needs of service users when required.
To assist in providing a purposeful, orderly and supportive environment for service users.
To support the promotion of positive relationships with parents, carers and outside agencies.
To work within the Bridge Centre policies and procedures.
To attend and undertake staff training specific to their role and the needs of service users and attend meetings as appropriate.
To take care of their own and other people's health and safety.
To be aware of the confidential nature of issues related to service users and any other outside agency, in keeping with the Data Protection Act (DPA) 1998 - ensuring that service users personal details are not disclosed to any unauthorised person and all records are maintained within the DPA 1998 requirements.
To respect the service users right to choose and refuse assistance. Service users are treated with respect at all times.
Support the service users to make informed choices.
Accidents and injuries are always reported, whether it be yourself or the service user, in accordance with the Bridge Centre NWL policy and procedure.
Person Specification
Essential skills, experiences and competencies:
Display a caring, compassionate and empathetic nature.
Good understanding of English both verbal and written skills.
Full clean driving licence and be willing to drive the minibuses.
Honest, trustworthy and polite.
Excellent time keeping.
Reliable, committed and responsible.
Excellent communication skills.
Ability to follow instructions.
Ability to cope under pressure.
Ability to remain calm and composed when faced with challenging situations.
Ability to work as part of a team.
Willing to undergo any relevant training to enhance your knowledge and practice and attend all mandatory training.
Undergo an Enhanced Disclosure & Baring Service check.
Qualifications and Experience
Diploma level 2 in Health and Social Care or be willing to work towards.
Experience of working with people with learning disabilities.
