The Jobholder is responsible for the management of a defined caseload and carries out continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.
The jobholder will provide specialist assessment care and advice to a specific patient group and will contribute to the development of the speciality service and will ensure cost effective use of resources.
Current services comprise providing specialist advice support and training to ensure high quality discharge planning is undertaken by wards staff.
Main duties of the job
Fulfils the requirements as directed by the Nursing and Midwifery Council or Health Care Professions Council to maintain and revalidate registration.
Clinical care delivery includes
Provide a clinical lead and role model to clinical staff by delivering high quality care.
- Undertakes clinical procedures where dexterity and accuracy are required.
- Maintain a safe working environment
- Acts in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies
- Undertakes duties on other wards or departments as and when required by service demands or patient need.
Support patients and their families communicating sensitive condition related information and ensuring they receive required information to enable them to participate in their care delivery
- Liaise with Community Services (as appropriate) to ensure seamless provision of care delivery
Education and Training:
- Provide training and education of learners and other junior staff
- Participates in own and others appraisals
- Provide education and training to patients and their carers.
- Keep up to date with developments, advances within the specialty
- Undertake mandatory and priority training within the required time scales
