As a Care Manager, you will be responsible for assist in managing and monitoring the quality delivery of a Home Care service for elderly and disabled individuals as an office and community-based care coordinator.
Duties of the job include:
Monitoring staff and client communications;
Monitoring and managing the rotas;
Responding to complaints and queries from clients;
Ensuring good record keeping;
Conducting Care assessments, and developing care plans and risk assessments.
Registering and interviewing Domiciliary Care Workers;
Conducting supervisions of Care Workers
Conducting quality assurance checks of Care Workers whilst out in the community.
Being part of Lillie-Roses 'On Call' system. Covering evenings and weekends
Emergency assignment of workers to cover care calls when required.
Key Skills:
Qualifed to a minimum of level 5 in Health and Social Care
Good mind-set;
Strong social care experience;
Be resilient and able to manage pressure;
Be a great team player with good communication skills;
Experienced in Rota planning using a staff roster software;
We will consider candidates who are looking to progress their career or have coordinated in other sectors;
Enjoy working as part of a team;
Be able to Provide excellent customer service;
Be able to Schedule Care Workers and able to put together rotas;
Be able to monitor care packages and care plans;
Knowledge and experience of CQC standards and CQC Compliance;
Knowledge of management systems and care coordination;
