Salary: 25,000 - 30,000 per annum (dependent on experience)

Job Type: Full-time, Permanent

Preferably a driver with access to their own car

Vivid Community Care is seeking a proactive and organised

Experienced Domiciliary

Care Coordinator

to join our growing team in the

London Borough of Hammersmith & Fulham

.

You will play a key role in ensuring the smooth day-to-day running of our care services by scheduling visits, supporting care staff, and maintaining excellent communication with clients and their families.

Key Responsibilities:

Organise and coordinate care staff rotas and daily schedules Ensure timely delivery of care in line with client care plans Communicate with care staff, clients, families, and health professionals Support the supervision of care workers Maintain accurate documentation and records

What We're Looking For:

Previous experience working in a similar

domiciliary care coordinator

setting Confident in using care management and scheduling software, with strong knowledge of care planning, scheduling systems, and CQC requirements Excellent communication and time-management skills Ability to multitask and work under pressure in a fast-paced environment NVQ Level 3 in Health and Social Care (or working towards) Strong team player with a positive and flexible approach. Committed to providing person-centred, high-quality care

Additional Requirements:

Must undergo an

enhanced DBS (Disclosure and Barring Service) check

Must provide

three years of references

Must successfully complete a

competency-based interview
