We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a pivotal role in ensuring that clients receive the highest quality of care. This position involves coordinating various aspects of client care, liaising with healthcare professionals, and managing administrative tasks within the office setting. The Care Coordinator will be instrumental in enhancing client satisfaction and improving overall service delivery.
Responsibilities
As an Experienced Care Coordinator, you will play a key role in managing and coordinating care services for individuals with learning disabilities, mental health challenges, and other complex needs. You will provide support to our dedicated care team and ensure that the highest standards of care are maintained.
Coordinate and manage client care plans, ensuring all services are delivered effectively and efficiently.
Liaise with healthcare providers, families, and clients to facilitate communication and address any concerns.
Maintain accurate and up-to-date records of client interactions and care plans within the office system.
Schedule appointments, follow-ups, and assessments for clients as required.
Assist in the development of policies and procedures to improve service delivery.
Provide support to clients by answering queries and offering guidance on available services.
Monitor client progress and report any changes or concerns to relevant stakeholders.
Participate in team meetings and contribute to the continuous improvement of care services.
Qualifications
Previous experience in a healthcare or social services environment is preferred.
Strong organisational skills with the ability to manage multiple tasks simultaneously.
Excellent communication skills, both written and verbal, with a focus on client interaction.
Proven experience in care coordination, preferably with learning disabilities, mental health services, or homecare.
Supervisory experience with the ability to lead and motivate a care team.
Strong understanding of care standards, CQC regulations, and best practices
Proficiency in office software applications (e.g., Microsoft Office Suite) is essential.
A compassionate approach towards clients, demonstrating empathy and understanding of their needs.
Ability to work collaboratively within a team while also being self-motivated.
