Duties



Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health or behavior Administer medications following prescribed care plans while adhering to HIPAA regulations for confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfers and mobility support Maintain accurate documentation of patient care, medication administration, and progress notes Support residents with memory care needs, including dementia and Alzheimer's care, ensuring a safe and engaging environment Conduct patient observation to identify any signs of distress or deterioration in health status Perform light cleaning duties to uphold hygiene standards within the care environment Assist with social activities and provide emotional support to promote well-being and engagement

Experience



Prior experience working in assisted living facilities, nursing homes, or group homes preferred Hands-on experience with home health or home & community care settings highly valued Knowledge of caring for individuals with developmental disabilities, autism spectrum disorder, or dementia required Certified in first aid and CPR; additional certifications such as HIPAA compliance are advantageous Familiarity with patient monitoring devices and techniques for vital signs assessment Experience with medication administration and creating or following personalized care plans Ability to lift heavy objects safely using equipment like Hoyer lifts or resident lifts Working knowledge of hospice care principles and long-term care practices beneficial Strong communication skills for working effectively with patients, families, and healthcare teams Demonstrated compassion, patience, and professionalism in caregiving roles
