26,409 - 27,711, plus 10% unsocial hours for qualifying shifts

Our Wiltshire Support at Home Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.


As a First Responder Support Worker, you will be supporting with Hospital discharges, assessing their needs, compiling a support plan, and attending to needs such as personal care, food preparation, and daily living activities as required. To meet the needs of the service, shifts include early morning, evenings, weekends, and bank holidays, traveling across the Wiltshire area to carry out customer visits.


We are seeking an empathetic individual with a genuine passion for helping others. A certificate in care or equivalent experience alongside knowledge of safeguarding and regulations relating to community care will be essential. To succeed in this role, you will need to showcase excellent communication skills and the ability to prioritise and organise your workload efficiently.
