Frailty Nurse
Job Description
Job Summary To continue to develop the frailty service for the PCN working closely with the PCN team and Practice staff. Participate in the care and development of anticipatory and current care planning for identified practice patients from all three practices in the PCN, to include treatment, preventative care, screening, future planning, and patient education in the practice and also in the patients home. To act as a focal point of communication between patients, doctors, nurses, and other members of the primary health care team within the practices and community teams at Dorset Healthcare.
