Delivery Programme - Connect to Work



Connect to Work is a new government-funded programme, part of the national Get Britain Working strategy. It is designed to support people with health conditions, disabilities, or complex barriers to employment to take up or stay in meaningful work.

In the North East, the programme uses the evidence-based Individual Placement and Support (IPS) model, helping people move into the right job quickly, receive tailored support in the workplace and build lasting careers.

Participation is voluntary and grounded in a person-centred approach, bringing health and employment services together to give people the joined-up support they need to thrive.

Are you passionate about building better systems for better outcomes? Subject to contract award, as our

Health & Employment Integration Lead

, you'll be the vital connector between local health services, community partners, and employment support, helping people with health conditions or disabilities access meaningful, supported work.

You'll work directly with GPs, PCNs, and health teams to develop referral pathways, shape joined-up care and embed the message that work is good for health -- not just in theory, but in real lives.

This is your chance to lead from the front, break down silos, and make inclusive employment a true part of recovery and wellbeing across County Durham.

Salary: 30,000-32,000



Location/travel: Durham with outreach required



Full-time and part-time applicants welcome



We actively welcome applications from candidates living in or near communities such as Woodhouse Close, Grange Hill, Todhills and Evenwood (DL14), Crook and Willington (DL15), and Spennymoor and Ferryhill (DL16).

A driving license and a car are essential to carry out this role.



What You'll Do:



Build relationships with health partners to generate referrals into Connect to Work*
Promote the health and wellbeing benefits of good work to clinical audiences - making the case for employment as a core component of recovery. Attend MDTs and co-locate with PCNs and community health teams Train and brief clinical staff on the benefits of employment and how to refer Share insight to improve integrated care and support continuous improvement Champion social value and inclusive employment at strategic forums and events Work closely with Employment Specialists and Occupational Inclusion Specialists to ensure customer journeys remain joined-up, person-centred, and reflective of clinical needs.

Your skills & experience:



Experience working in or alongside primary care or community health Strong relationship-building and influencing skills A strategic mindset with hands-on delivery experience Passion for inclusion, co-production, and systems that serve people better Confidence to work independently across community and clinical settings

Desirable



Background in public health, social work, occupational therapy, or employment support. Understanding of the Individual Placement and Support (IPS) model or supported employment approaches. Experience engaging with Integrated Care Boards (ICBs) or similar system-level health bodies.
