The purpose of this role is to provide a generic housing-related floating support service in Caerphilly for People aged 18+. Support will be person centered, focusing on realistic agreed goals and appropriate outcomes.
KEY RESPONSIBILITIES
To provide reactive, innovative, and specialised crisis-related housing support to people aged 18+ across Caerphilly Borough.
To work with people who may have several complex support needs including, mental health, alcohol and substance misuse, anti-social behaviour issues, hospital admissions, alongside experiencing homeless or are at risk of losing their tenancy.
Complete comprehensive, person-centred support plans with service users focusing on realistic agreed goals which enable outcomes to be monitored
To log clear and concise information on shared IT Systems ensuring all electronic case notes are completed accurately and to a quality assurance level standard
To perform routine administrative tasks as directed by the Housing Manager including keeping detailed records in order that accurate statistics can be compiled
To assist service users with support in their home, office or on the telephone as required
To have a clear understanding of housing needs of people aged 18+ and, in particular, those adults at risk or in crisis situations
Provide housing-related support to service users in a variety of ways including financial maximisation, practical skills, maintaining payments and promoting economic progress, improving resilience, accessing appropriate services and accommodation
To be mindful of complementary services available in Caerphilly and signpost to alternative services to meet needs
To help vulnerable people develop, or maintain, the skills and confidence necessary to live independently
To maintain confidentiality and understand issues of General Data Protection procedures
To be willing to work across the service's operational area
To submit expenses claims, annual leave requests and all other HR matters through an online portal
To be willing to undertake ongoing and relevant training both in person and via e-learning
To form an active part of the team, participating in team meetings and attending regular supervisions and appraisals.
Willing to work flexibly to meet the business need as and when required between 9.00am and 5.00pm Monday to Friday.
Full driving license and access to a vehicle for work purposes
Willing to undertake any relevant training
