We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will possess strong interpersonal skills and a commitment to supporting individuals in need. As a Case Worker, you will play a crucial role in assessing client needs, developing tailored support plans, and advocating for resources that enhance the well-being of those you serve.
Duties
Conduct comprehensive assessments of clients to identify their needs and challenges.
Develop and implement individualised support plans in collaboration with clients and relevant stakeholders.
Provide ongoing support and guidance to clients, ensuring they have access to necessary resources and services.
Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
Facilitate communication between clients and service providers, advocating on behalf of clients when necessary.
Monitor client progress and adjust support plans as needed to ensure effective outcomes.
Negotiate with service providers to secure appropriate services for clients.
Participate in team meetings and case reviews to share insights and improve service delivery.
Qualifications
A degree in Social Work, Psychology, or a related field is preferred but not essential.
Previous experience in case management or social services is advantageous.
Strong negotiation skills to advocate effectively for clients' needs.
Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
Ability to work independently as well as part of a team in a fast-paced environment.
Strong organisational skills with attention to detail in record keeping.
Empathy and a genuine desire to help others improve their circumstances.
