Specialist Memory Assessment & Dementia Support Service (SMADSS)

Following the recent merger of Memory Assessment Services and OA Day Services, we are now operating as SMADSS. We are seeking an enthusiastic Band 6 Lead Nurse to join our dynamic and welcoming team based in North Derbyshire.



Our service runs Monday to Friday, 9:00am-5:00pm (excluding bank holidays). This is a full-time, permanent position of 37.5 hours per week.



Carry out comprehensive and specialised initial memory assessments for individuals experiencing memory or cognitive difficulties that may be associated with dementia.



Apply person-centred and compassionate approaches , actively listening and responding to individuals to encourage their engagement with the Memory Assessment Service (MAS).



Use a robust framework for risk assessment, safeguarding, signposting, and referrals to specialist teams, ensuring collaboration with the individual throughout the process.



Provide support across Derbyshire , delivering services in a variety of settings to meet the needs of the Memory Assessment Service.
