We are looking for a Nurse Practitioner to join our Team.

As part of a multi-disciplinary team, we aim to assess, diagnose, treat and manage the needs of our patient demographic to improve the health of the local community. Working in our modern purpose-built medical centre with a diverse practice population of approx. 19,000, you will never be bored.

Our Nursing team consists of 5 HCA's, 5 Practice Nurses and 3 Nurse Practitioners managed by a Nursing Team manager and overseen by a Clinical Services Manager. We are looking for a new Nurse Practitioner to join our Nursing team for between 34 and 37 hours a week.

As a practice we provide teaching for medical students, participation in research studies and participation in service transformation pilots.

Job Summary:



Working autonomously, to triage, diagnose and treat patients presenting with acute illness or injury and to advise patients in respect of their continuing medical and nursing needs.

Job Requirements:



Offer face to face assessment and telephone triage of patients with acute illness or minor injuries.

Complete E-consults within your remit

Complete weekly telephone care home ward rounds

Assess, diagnose and treat patients presenting with acute illness, all ages (8 weeks plus).

Independent Prescriber (essential)

Advise patients on general health care, arranging investigations or referring to other members of the primary health care team as appropriate

Assess and treat minor injuries

Work in accordance with Practice protocols, PGD's and national or local guidelines.

Participate in the induction and training of GP Registrars, Students, Nurses or Staff

Provide nursing cover where necessary and undertake home visits if required

Ensure patients' electronic records are updated immediately and appropriately following the consultation

Assist with our GSF (Gold Standards Framework) register, completing reviews, referrals, signposting and symptom management to ensure there are no gaps in the patient care.

Suggested hours (TBC)



Monday: 08:00-18:00 (9.5 hrs)

Tuesday: 08:00-17:00 (8.5hrs)

Wednesday: Off

Thursday: 08:00-18:00 (9.5 hrs)

Friday: 08:00-18:00 (9.5 hrs)

Salary:

From 24.85 per hour
