Job Purpose
To support older people, particularly those affected by dementia and their family carers, by providing high-quality information, advice, emotional support, and meaningful group activities. This role is key in helping individuals maintain wellbeing, build resilience, and feel supported within their community.
Key Responsibilities
Deliver one-to-one dementia information, advice, and emotional support to clients and their families.
Attend and support the weekly Memory Cafe, offering a welcoming presence and sharing advice and information.
Facilitate carers groups, providing a safe, supportive environment for discussion and mutual support.
Support volunteers to deliver our Living with Loss group - for Carers of People with Dementia to support those adjusting to changing relationships and circumstances.
Other Duties
Maintain accurate and confidential client records, tracking outcomes and impact.
Liaise with health and social care professionals, ensuring joined-up and appropriate support for clients and carers.
Contribute to internal evaluations, data collection, and impact reporting.
Support funding bids and service development by providing insight into client needs and feedback.
Operate in accordance with all TC policies and procedures.
Maintain strict confidentiality of clients.
