Overview

The Operational Lead position will play a key role in providing expert operational leadership in conjunction with the Associate Director and partner organisations across all pathways in delivering on safe and timely discharges, ensuring quality of care.

The post holder will be accountable for the provision of effective professional leadership and pro- active resource management for all relevant pathways. This includes the Transfer of Care Hub (ToCH) Virtual Wards (VW) and Community Bed Based Care (CBBC).

Responsibilities

Develop detailed and complex technical understanding of discharge to assess pathways and processes to lead service delivery from a point of professional expertise and competence. Work with the COO and ADs in providing assurance, safe, and effective service delivery, including responsibility for data reporting. To deputise for the ADs where necessary including attendance at required system or Alliance forums. Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity. Develop high quality team working, implementing target setting where appropriate by agreeing standards within the discharge-focussed multi-disciplinary teams. Develop and implement systems for audit, quality assurance and outcome measurements. Performance manages direct reports and enables personal and professional development which underpins the strategic direction of the service and CSH

Contract length: 5-6 months

Pay: 32.49-38.82 per hour

Expected hours: 37.5 per week
