We are seeking a dedicated and proactive Outreach Worker to join our organisation. The successful candidate join the Substance Misuse Floating Support Team, as well as working across other floating support projects when required.
You will be working with a caseload of individuals providing support at properties across Darlington. The role involves working within a team but you will be essentially lone working and providing intense outreach support to individuals who have issues such as failing to sustain accommodation, substance misuse, mental ill-health, and who may present with challenging behaviour. The aim is to support individuals to stabilise their situation, assisting them to maintain their tenancy and preparing them for living independently.
Outreach support may include supporting people who do not have accommodation so interagency working is required to ensure a holistic approach to an individual's support needs.
Staff will also be involved in an out of hour's on-call rolling rota.
Candidates must have prior experience in supporting individuals with complex needs and lone working.
Duties
Understand and implement a smooth transition for individuals into floating support services, and complete all relevant paperwork.
Ensure that support meetings are conducted in a caring, considerate, efficient and professional manner at all times.
Liaise with both the Manager and the Local Authority Housing Options Team for clients both entering and leaving the services.
Liaise regularly with all agencies involved with the individuals support.
Work closely with each individual on caseload, on a one to one basis, to implement an agreed support package, leading to the acquisition of permanent, independent accommodation.
Provide both Tier 1 and 2 brief interventions for drugs and alcohol and refer into Tier 3 services. To recognise when clients require further professional support and refer to specialist services.
Refer to appropriate specialist agencies for any identified needs and, where appropriate, accompany individuals to engage with statutory and voluntary agency meetings.
To encourage the building of strong relationships and support networks.
To ensure that at exit, each client has an appropriate package on on-going support to maximise the chance that independence will be sustained.
