About The Role





About the Preston Outreach team



We work with care leavers, asylum seekers, NHS patients, and families, providing comprehensive support within the community.



For young care leavers, our focus is on facilitating a smooth transition into adulthood, empowering them with essential independence skills. This includes preparing them to take on a tenancy or move into social housing, enabling them to confidently live on their own.



In the case of Separated Children, we assist in their settlement process, help navigate immigration procedures, support their integration into college life, facilitate access to healthcare services, and ultimately guide them towards complete independence.



For adult NHS referrals dealing with complex mental health issues, our dedicated team provides community-based support. We work closely to address their specific needs and contribute to their overall well-being within the community setting.



Due to the areas this role covers a car driver with a vehicle is essential.



About you


The ideal candidate will be passionate about this sector, looking to make a difference with our service users and will be looking for a challenging but rewarding role.



Essential


Previous experience supporting (not caring for) people, families or young people to gain independence. Travel is essential so must hold a full driving licence and have access to a vehicle insured for business use. Thorough knowledge of safeguarding vulnerable adults and children Have or be willing to work towards an NVQ Level 3 Health and Social Care qualification. Computer literate Good interpersonal skills

Key Duties:


Managing a caseload of Service Users, helping to build independent living skills

Liaising with relevant professionals such as Social Workers, Healthcare practitioners and the Police

Writing detailed reports which assess a service user's independent living skills

Completely weekly inspections of Service User's properties

Support with accessing and maintaining education/employment

Promote physical, mental and emotional well-being, de-escalating challenging situations when they arise

Signposting to appropriate services as required, including mental health, drug rehabilitation services, counselling and education
