Location:

Based at Head Office The Youthy & Community Hub, Thornaby Road, TS17 6LS, Home Working and attending customer homes within the borough of Stockton-On-Tees

Hours:

Average of 22.25 hours per week + 1 weekend duty

Responsible to:

Registered Manager

Salary Scale:

31,221 per annum (pro-rata) -

Actual Salary 18,774.79 + 350 flat rate for weekend duty



Contract Period

: 9 months (maternity cover)

Job Purpose:

To provide high quality coordination of person centred homecare services that support customers with their choices to live independently in their own homes.

Key responsibility



Efficiently coordinate the day to day running of the business. Allocate resources and monitor performance to deliver high quality homecare to customers within budget.

Duties and specific responsibilities



Manage the safety and quality of the business



Responsible for the planning and implementation of rosters, using an electronic rostering system to deliver timely services to customers, allocating workload accordingly to Care Assistants including arranging cover due to sickness/annual leave etc.

Contribute to efforts to monitor, evaluate, and report on social impact outcomes

To be the first point of contact for employees, customers and their families

Ensure customers receive continuity of care by ensuring (where practical) they have the same Care Assistant visit them. You will be responsible for ensuring that the customer knows who their personal care team are.

Ensure all emergency out of hours issues are dealt with effectively, such as covering calls either directly or indirectly when care workers are sick or absent

To work alongside the Registered Manager to agree and achieve growth targets for your delegated area.

To work in partnership with other professionals, customers and their families to enable the customer to remain in their own home

To attend review meetings, as required, to ensure assessed needs of customers are understood

To support the Registered Manager in CQC and Local Authority inspections

To follow all relevant statutory policy, codes of practice and procedure guidelines associated with the operation of the service

Deputise for the Registered Manager as and when required

To ensure the maintenance of adequate and appropriate records, key performance indictors and information systems in respect of finance, customers and the work place as required by Senior Management Team, Care Quality Commission, Commissioners and Internal Audit

To take reasonable care of your own health and safety and cooperate with Senior Management Team, so far as necessary, to enable compliance with health and safety rules and legislative requirements

Undertake training and development to keep up to date with the law, best practice and changes in company policy. Apply this knowledge to day to day coordination and delivery of care

Be prepared to work flexibly to ensure the safe delivery of the service

Lead and coordinate staff



Daily supervision of Care Assistants to ensure care and support is delivered to customers Support the Health & Social Care Manager in respect of employee related matters including:
o Information to payroll

o Sickness absence monitoring

o Return to work meetings

o Induction of new employees

o Training and development including NVQs/medication etc

Provide a good service to customers



To undertake customer contact visits to ensure appropriate service delivery To ensure that all customers both internal and external, receive a consistently high-quality level of service, commensurate with the standards required by Five Lamps Promote the rights of each customer and keep their wishes at the centre of their care and support Make sure that prior to each service commencing, a customer assessment and risk assessment with the customer, and/or their chosen representatives, has been completed including what the customer needs and would like to achieve from their care and support
Make sure a written individually tailored care and support plan has been created and agreed, that respects the customer's wishes and promotes their dignity and privacy. Agree appropriate risk control measures to reduce identified risks

Apply excellent communication skills with customers, their families and representatives, staff and other health and social care professionals to deliver high quality homecare services

Promote the business



Attend external meetings and represent the service in a positive manner

Participate in the growth and development of the business by attending promotional events and through word of mouth

To monitor and maintain quality throughout the different areas of the service in relation to the external quality standards and inspection bodies and organisations

To attend care calls during critical periods when sickness levels are high

To ensure service users receive the highest standards of care

To take reasonable care of your own health and safety and cooperate with Senior Management Team, so far as necessary, to enable compliance with health and safety rules and legislative requirements, this includes being up to date with Covid vaccinations as required by Government legislation.

To undertake any other duties and responsibilities as may be reasonably required within the scope of the post. We will provide full training in line with regulatory requirements.
