About The Collaborative Care Team

PML's Collaborative Care Team (CCT) supports seventeen General Practices located within West Northamptonshire, providing proactive assessment and support to people with long-term and complex conditions and their Carers. We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital.

The Team currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), and a Social Prescribing Link Worker. The CCT operates Monday to Friday, 08:30 to 16:30 excluding Bank Holidays

Additional Accountabilities

Accountability 1

To provide daily coordination of the CCT service delivery under the direction of the Clinical Lead, acting as a central

point of contact to ensure appropriate support is made available to the team, patients and their carers.

Accountability 2

To regularly liaise with the range of multidisciplinary professionals and colleagues involved in a person's care,

facilitating a coordinated approach and ensuring everyone is kept up to date so that any issues or concerns can be

appropriately addressed and supported. This includes actively participating in multidisciplinary team meetings

within the CCT, PCNs and Practices

Accountability 3

To provide administrative coordination for the CCT service delivery supporting PML Governance, Operational and

performance compliance.

Accountability 4

To develop expertise in the administrative functions of Clinical systems, specifically Systemone and EMIS, including

problem solving, staff support, access, reporting and contingency.

Accountability 5

To keep accurate and up-to-date records of contacts, appropriately using the CCT clinical system, GP and other

records systems relevant to the role, adhering to information governance and data protection legislation. To

support the Clinical Lead to ensure that PML/CCT remains safe and effective, including using audits and learning

from significant events and complaints including the collation of evidence for CQC compliance.

Experience Requirement

Working within a health and or social care environment and with clinical systems

Desirable

Working as part of an integrated multi-skilled team to optimise patient care and

experience. Desirable

Skills & Knowledge Requirement

Good interpersonal skills; the ability to make effective relationships with others.

Able to work in a changing environment; demonstrates an ability to cope under

CCT Office based at Slapton, Towcester with travel to GP Practices and PCNS. Some home working.

Job Type: Full-time

Pay: 25,000.00-26,000.00 per year
