PATIENT CARE COORDINATOR



We are seeking a compassionate and organised

Care Coordinator

to join our reception team in this vital front-of-house role. As one of the first points of contact for many patients, you will play a key role in improving access to care by helping to guide patients quickly and effectively to the right professional or service.

With comprehensive training, you will support the clinical triage process by helping to identifying the most urgent appointment requests and ensuring they are prioritised appropriately. You will also provide essential assistance to patients with additional needs or those requiring reasonable adjustments, helping to ensure this information is clearly visible on the clinical system and online consultation tool to enable the practice to provide an inclusive, and supportive environment for all.

Your excellent communication skills and empathy will enable you to understanding patient needs and directing them with confidence. Working closely with the duty doctor and the reception team you will contribute to the smooth running of the practice and help enhance patient experience.

Job summary



The post holder will assist patients, carers and other organisations with making appointments, accessing care and directing them to the appropriate service or healthcare professional in a courteous efficient and effective way. This will be either in person, or via our online triage tool.

tThe Role will include working as part of the wider team and will involve a mixed variety of duties which includes care navigating, working as part of the appointment triage team allocating appointments most clinically urgent, under supervision of the duty GP, reception duties and general admin. The practice is large and busy so will suit a person who is use to a fast paced environment. Whilst supporting patient care, much of the role is via our online consultation tool or via telephone.

If you enjoy helping people and listening to them with the intention of trying to figure out how you can assist them to access the services they need, then this role is for you. This means that you are not only supporting our patients, but our large clinical team too.

You will need to be digitally savvy and also comfortable with change since the NHS is required to constantly change to meet the emerging needs of our population.

Main duties of the job



The post holder will be responsible for reviewing income requests for appointments, meeting patients' needs in the most efficient manner which often includes directing them to colleagues in-house, who have a wide range of skills or signposting them to many of the excellent community services available to us all in the surrounding area.

Some of the time we are unable to offer exactly what a patient wants, exactly when they want it, but it is the way in which you communicate what we can offer which will make you a good patient care coordinator.

The successful applicant will be able to demonstrate a mature outlook on life and have strong work ethics and good problem solving skills. The role would appeal to someone who likes to be organised and thrives in a fast-paced environment.

About you



You must be caring and empathetic whilst maintaining our excellent standards.

Good IT skills are essential.

Ideally you will have experience within a GP surgery and knowledge of EMIS Clinical software.

You must be an approachable, patient centered individual, very organised with good attention to detail, caring and patient, a team player and punctual.

Job responsibilities



To provide a high standard of customer service on behalf of the practice, in support of the rest of the practice team.

To adopt a skillful approach to communication in all circumstances.

To perform the role of care navigator to our patients.

Expected hours:



32- 37 hours across Monday - Friday. Must be able to cover a shift pattern during core hours of 8:00 - 18:30.

Job Type: Full-time

Pay: 24,500.00-26,000.00 per year

Ability to commute/relocate:

Sale M33 7SS: reliably commute or plan to relocate before starting work (preferred)
