Job Description

We have an exciting opportunity for a Pharmacist to join our team at The Wick Surgery, on a one year fixed term contract.
The Surgery is based in Hackney and has a growing list size of over 8000 patients.
Within this role you will work closely with the multidisciplinary clinical team of GP's Physician Associates and Pharmacists. The role involves delivering medication reviews, minor ailments clinics, QOF and LTC targets and facilitating safer prescribing meetings.
This role requires clinical experience, and would suit an applicant who has an independent prescriber qualification.
Main duties of the job

• Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
• Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage
• responsibility for day-to-day medication reviews
• delivering prescribing aspects of the Investment and Impact Fund.
• Answer queries from the non-clinical members of the team
• Provide support to the practice repeat prescribing systems and medicines reconciliation. Support the implementation of MHRA

• support patients to take their medications to get the best from them, reduce waste and promote self-care
• Support CQC compliance.

• Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network.
• Help when needed in signing off global repeat prescriptions

• To complete QUIPP work and other practice-based projects in line with set guidance from the ICB Medicine management team.

About us
Friendly GP Surgery in East London looking to recruit Pharmacist on a fixed term one year contract to provide excellent customer care to our patients and carry out a variety of tasks.
Our team includes 1 GP principal, 5 salaried GPs, 2 Physicians Associates, 1 PCN Pharmacist, 1 Prescribing Pharmacist, 1 Sexual Health Nurse, 1 Practice Nurses, and 2 Health Care Assistants.

• Clinical medication reviews for patients with single or multiple long-term conditions where medicines optimisation is required
• To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission.
• To be involved in multidisciplinary clinical meetings within the Practice and PCN where appropriate
• To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate.
• Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate
• Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary
• To identify areas of clinical risk at network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources.
• To maintain own clinical and professional competence and be responsible for own continuous professional development
• Work with the practice to ensure full compliance with Care Quality Commission standards for safe and effective care
• To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required.
