Job Description

We are looking for an experienced and values-led Physiotherapist to join our dedicated Under-65s unit, Evergreen Highfield Hall on a part-time basis of initially 8-10 hours per week (flexible work pattern). Working as an integral part of our multidisciplinary rehabilitation team, you will play a vital role in supporting adults of working age who are unable to live independently due to life-changing illness or trauma.

You will work closely with OTs, therapy assistants, wellbeing specialists, nurses and carers to deliver goal?based, holistic rehabilitation that empowers residents to shape their routines, build autonomy, and participate fully in a vibrant, community?led home environment.

Where appropriate, you will also contribute to longer?term plans that support residents to move towards greater independence or more independent living.

KEY RESPONSIBILITIES

1. Rehabilitation Planning & Delivery

Conduct physiotherapy assessments for residents with a range of physical, neurological, MSK or trauma-related conditions, creating individualised treatment plans rooted in the principles of autonomy, independence and holistic rehabilitation. Provide regular one-to-one physiotherapy sessions (planned at 3-5 per week per resident as appropriate) and functional rehabilitation tailored to each person's goals. Support residents to regain, develop or adapt skills such as mobility, transfers, balance, strength, coordination, and daily living activities. Promote active participation in everyday routines and encourage residents to explore hobbies, education, outdoor activities and community engagement as part of their rehabilitation trajectory. Use assisted technologies where relevant, supporting independence and confidence.
2. Multidisciplinary Collaboration & Coaching

Work closely with OTs, therapy assistants, wellbeing team, nurses and carers to develop cohesive, person-centred rehabilitation plans. Participate in MDT goal-setting meetings, ensuring resident voice and choice influence all decisions. Provide coaching, guidance and hands-on training to the care team to ensure rehabilitation principles are embedded into day-to-day interactions (e.g. safe mobility support, pacing, functional routines). Liaise with external health professionals, community partners and relatives to support continuity of care and safe progression towards independent living where appropriate.
4. Documentation, Reporting & Digital Systems

Maintain accurate and timely clinical records, progress notes and risk assessments using our digital platforms as appropriate. Monitor and evaluate progress against goals, adapting physiotherapy plans as needs evolve. Contribute to compliance reporting and preparation for inspections where required through appropriate documentation.
5. Safeguarding, Quality & Professional Standards

Uphold all safeguarding policies and act swiftly to protect residents where concerns arise. Ensure physiotherapy activities and manual handling practices meet CQC and professional standards. Maintain a safe therapeutic environment, demonstrating strong clinical reasoning and risk management. Commit to continuous professional development, evidence-based practice, and reflective learning.
REQUIREMENTS OF THE ROLE

Qualifications & Experience

Degree in Physiotherapy and HCPC registration. Experience with adults with neurological, MSK, trauma-related or complex needs. Familiarity with rehabilitation and goal-based therapy models. Comfortable using digital care systems (training provided).
Skills & Knowledge

Strong clinical assessment and risk management skills. Ability to deliver holistic, person-centred therapy. Understanding of autonomy-focused rehabilitation principles.
