We are seeking a Practice Nurse for up to 20 hours per week, to be worked over two days, at Barton House Medical Practice. The successful post holder will operate within their own sphere of competence and agreed acceptable limits of practice. The PN will need to prioritise and triage the needs of patients accordingly with use of knowledge and competencies as a qualified PN. Main duties of the job
The role will involve working as part of the multi-disciplinary team to deliver safe and high-quality care. The practice nurse duties will include (amongst other): Chronic disease management Cytology Basic wound care ECGs INR monitoring Routine and childhood immunisations. Overview of the organisation
We are a rural GP practice, in the beautiful location of Beaminster with a fun, happy, hardworking, and resilient team. We are well supported with strong relationships with the community primary care team including a community rehabilitation team, community matron, district nurses. Our clinical team consists of supportive GP's, ANP's Practice Nurse and Health Care Assistants. With a strong management team who are always on hand to help and advice, as well as a friendly nursing team, administration, reception, and dispensary teams. Job Summary
To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice, and will be expected to deliver health promotion as required and chronic disease management in line with their experience.
Responsibilities
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
To work to the IPC policy
Develop implement and embed health promotion and well-being programmes.
Identify, manage, and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
Provide wound care (ulcer/doppler etc) to patients.
Processing and requesting pathology services/results as required.
Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
Implementation and evaluating individual treatment plans for chronic disease patients.
Maintain accurate clinical records in conjunction with extant legislation.
Ensure read codes are used effectively.
Assist in maintaining chronic disease registers.
Chaperone patients where necessary
Prioritise health issues and intervene appropriately.
Support the team in dealing with clinical emergencies.
Recognise, assess, and refer patients presenting with mental health needs to the GP.
Implement vaccination programmes for adults and children.
Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
Work as part of the clinical team with all safeguarding matters in accordance with local and national policies
Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
Deliver opportunistic health promotion where appropriate.
Work as part of the clinical team providing minor illness service (if in scope of practice)
Secondary responsibilities
In addition to the primary responsibilities, the practice nurse may be requested to:
Monitor and ensure the safe storage, rotation, and disposal of vaccines.
Support members of the nursing team including Health Care Assistants and Phlebotomists with assistance where necessary
Participate in local initiatives to enhance service delivery and patient care.
Support and participate in shared learning within the practice.
Continually review clinical practices, responding to national policies and initiatives where appropriate
Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)
Essential criteria
Registered Nurse Evidence of post registration study
Experience of working in a primary care environment
Experience of working as a practice nurse
Desirable criteria
Experience of Chronic disease management, NHS Health Checks, Contraception Support

In the course of seeking treatment, patients entrust the practice with, or allow the practice to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
In the performance of the duties outlined in this , the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source however stored is to be regarded as strictly confidential and staff should only access information held in the Practice for a valid purpose and only appropriate to their job role and will be required to sign an agreement to this effect.
Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in the proper course of your employment or as required by legislation.
