Job Summary

We are seeking a skilled and compassionate Practice Nurse to join our healthcare team. The candidate must be able to work all day (til 18:30) Monday and/or Friday.

Responsibilities

Work as part of the Practice Nursing Team Provide assessments, screening, treatment services, and health education advice Manage leg clinic and facilitate leg management pathway Prepare/assist/facilitate vaccinations and immunisations Undertake collection of pathological specimens Provide appropriate follow ups for patients Lead in an area of chronic disease management Ensure accurate notes are recorded Attend and participate in practice meetings as required Assist in formulation of strategy and policy Undertake administrative procedures Participate in collection of stats for audits Opportunity to be involved with our COVID Medicine Delivery Unit providing infusions

Requirements

2+ years of practice nursing experience Experience in chronic disease management Knowledge of needs of patients with long-term conditions. Knowledge of health promotion strategies. Awareness of accountability of own role and other roles in a nurse-led service. Awareness of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Clinical skills cervical cytology, immunisation and vaccination, ear care. Change-management skills and ability to support patients to change lifestyle. Good communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. EMIS Web clinical system experience. Management of long-term conditions. Involvement in implementing and using protocols and clinical guidelines. Experience of audit.
Job Type: Part-time

Pay: 17.00-24.00 per hour

Expected hours: 16 - 24 per week
