1. To assist with the assessment, planning and implementation of agreed care in relation to the management of long-term conditions, including annual and follow up health reviews specifically for this post, the following (some of and not necessarily all of - ) a. Diabetes b. Asthma c.
COPD d. Coronary heart disease e. Heart Failure f. Hypertension Management g.
Rheumatoid arthritis monitoring h. Family planning i. QOF areas learning disability checks and mental health reviews (Additional training will be arranged if required in these areas.) 2. To advise patients on general health care with referral to GP as necessary.
3. Perform a holistic assessment of patients attending for a range of Practice nursing duties, including (but not exclusive) a. Cervical screening b. Wound care / ulcer care/Doppler including compressions bandaging c.
Removal of Sutures d. Well man/woman checks e. Travel advice and immunisation f. Contraception and womens health advice, including HRT.
g. Venepuncture h. Routine immunisations/Childhood immunisations i. Chaperoning and assisting patients where appropriate who are being examined by another clinician j.
Assisting with minor surgery and coil fittings, as requested k. Requesting pathology tests, for example urine culture, swabs
