We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The ideal candidate will play a vital role in providing high-quality patient care within our practice. You will be responsible for supporting patients through various stages of their healthcare journey, ensuring they receive the best possible treatment and support.
Duties
Conduct patient assessments and record medical histories accurately.
Administer vaccinations and medications as prescribed.
Assist in minor surgical procedures and wound care.
Provide health education and advice to patients regarding lifestyle changes and disease prevention.
Collaborate with other healthcare professionals to develop and implement patient care plans.
Maintain accurate patient records in compliance with confidentiality regulations.
Monitor patient progress and report any changes to the medical team.
Ensure that all clinical equipment is maintained and used safely.
Skills
Delivering exceptional levels of care to all patients
Conducting health assessments and screening programs
Working autonomously and as part of a team
Administering immunisations and vaccinations to both children and adults
Providing wound care and dressing services
Offering advice on family planning and sexual health
Carrying out cervical cytology
Maintaining accurate patient records and documentation,ensuring confidentiality and adherence to GDPR guidelines
Referring patients to secondary/specialist care as required
Preferred experience:
Experience in chronic disease management - Training in Diabetic management, Asthma and COPD would be desired.
Experience using SystmOne.
Strong knowledge of patient care principles, with a focus on delivering compassionate support.
Proficient understanding of human anatomy to assist in effective patient assessments.
Excellent communication skills, both verbal and written, to interact effectively with patients and colleagues.
Ability to work independently as well as part of a multidisciplinary team.
Strong organisational skills with attention to detail in record keeping and reporting.
