Maritime Health Partnership is a leading Primary Care provider operating a network of GP Surgeries across Maidstone and Medway Kent. We are innovative in our approach and are known for providing high quality Primary Care services to the local population.

We are seeking a dynamic, resilient, Practice Nurse with a special interest in diabetes to join our friendly, motivated, and mutually supportive nursing team. Our multidisciplinary team consists of GPs, ANP, Clinical Pharmacist, Physician Associates, Specialist Chronic Disease Nurse, Paramedic, Mental Health Practitioner and HCAs, together with a strong and experienced management and admin team able to support the delivery of excellent care to our patients.

The post holder will be flexible, hardworking and empathetic and responsible for the delivery of practice nursing activities to the practice population. They will work collaboratively with the Nurse Lead and general practice team to meet the needs of patients, supporting the delivery of quality QoF and enhanced services, maintaining safe infection control processes, supporting the delivery of immunisation (both child and adult) and cervical screening programmes, safeguarding, policy and procedures, and providing nurse leadership duties as required.

You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the Lead Nurse and Partners to deliver organisational priorities in our Practice locations in Essex.

Clinically, the focus of the role is management of patients with long-term conditions and general nursing interventions for all patients with experience in Primary Care and the knowledge, skills and with evidence of training in Diabetes management. You will be pivotal in managing patients diabetic regimes, offering advice, support, medication changes along with supporting the Practice with its targets and maintaining our high standards of care.

Participate in the administrative and professional responsibilities of the practice team, ensure accurate notes of all consultations and treatments are recorded in the patient notes.

Note:

Successful candidates are required to provide documented evidence of Full Immunisation History as a condition of deployment. This is a standard requirement for individuals working in a Care Quality Commission (CQC) regulated health or social care workplace unless exempt.

This post is subject to an enhanced DBS disclosure and relevant occupational vaccination.
