An exciting opportunity as arisen for a practice nurse at Llanfair Surgery! We are a friendly rural practice serving 5,600 patients over two sites. Our main surgery is Llanfair Surgery, Llandovery with a branch practice in Langadog. You'll work alongside a dedicated and experienced nursing team. We are a patient and community-focused team, committed to delivering high-quality care.

All registered adult nurses with an interest in primary care and chronic disease management are encouraged to apply. Regardless of experience, you will be trained and equipped with the necessary skills and education to carry out the following job description.

JOB PURPOSE



To undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs and the practice nursing team or independently, in line with competence and need.

To maintain or improve standards of care and be actively involved in identifying practice population need.

Maintain care home register carrying along with associated clinical tasks and data management.

To provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health.

Carry out chronic disease reviews and advise patients in relation to COPD, asthma, diabetes and chronic heart disease.

To help provide education and training regarding chronic clinical conditions as required.

To undertake any further training required to ensure appropriate role development.

Maintain accurate records using EMIS clinical software along with associated systems.

To practice within the NMC Code of Professional Conduct-Prioritising People, Practising Effectively, Preserving Safety and Promoting Professionalism and Trust.

PROFESSIONAL RESPONSIBILITIES



o Assist practice GPs and nursing team to deliver effective and holistic management of patients with chronic diseases in line with best practice and Welsh Government/Health Board expectation

o Carry out data quality tasks to ensure correct coding as well as undertaking searches as requested/required.

o Contribute to the development of policies / protocols / guidelines within the practice.

o Aid the practice team with implementation of new initiatives in the delivery of chronic disease management/enhanced services.

o Formulate reports as required for the Practice Manager and wider clinical team.

o Support the implementation of evidence-based guidelines in conjunction with the practice clinical team.

o Involvement and leadership in the process of Enhanced Services administration, including reports/searches.

o Promote the continuing professional development and skills of both yourself and other nurses within the practice.

LEADERSHIP



o Contribute professionally to the practice nursing team.

o Offer guidance and support to the practice nursing teams to maintain/improve standards of care.

o Work alongside the nursing team in chronic disease management within the practice & provide practical assistance and professional advice to achieve the highest standards of care.

o Provide clinical input alongside other practice nurses, to assist in skill development and sharing of best practice.

o Delegate to others with appropriate skills, knowledge and competence.

o Maintain nursing practice in line with relevant evidence-based research.

o Act as a resource to the general public, patients within the health care system, their families, carers and external healthcare professionals.

o Attending and contributing to a range of practice meetings including, palliative care, child protection, multidisciplinary and significant events.

o Practice according to the NMC Caring with Confidence Initiative-The Code in Action by: practising accountability, using professional judgement, delegation and speaking up.

CLINICAL ROLES



Primary Roles:



o Leadership and involvement in the design and delivery of chronic disease management reviews including COPD, asthma, diabetes and CHD clinics.

o Contribute to the maintenance of the practice disease registers.

o Provide advice on lifestyle issues, assist and support patients in changing their behaviours and refer appropriately.

o Undertake accredited training/updates in chronic disease management.

o Work alongside and support wider nursing team to implement and deliver vaccination/immunisation programmes, working to Patient Group Directions.

o Carry out nurse-led care home reviews and ward rounds in line with Enhanced Services specification alongside and supported by Lead GP.

o Work alongside other practice staff to help develop their knowledge and skills.

o Support the practice in achieving Enhanced Service and Welsh Government/Health Board targets.

o Undertake opportunistic screening when clinically required/indicated.

o Performing venepuncture and electrocardiography where required.

o Undertaking wound care management.

o Undertaking family planning assessments, provide advice and refer patients appropriately.

o Performing cervical screening (where qualified).

o Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.

o Deliver travel healthcare, including vaccinations and advice for patients.

Research, Audit and Data Quality



o Support the practice audit strategy, action plan and evaluation.

o Contribute to the provision of equitable, high-quality services for patients in the community.

o Help support the integration across primary, secondary and tertiary care services.

o Maintain an interest in national research initiatives and disseminate appropriate information to health professionals and patients as required.

o Ensure all care is evidence based using up to date validated literature.

o Engage and contribute to quality improvement projects.

o Carry out data searches when required/requested.

o Involvement in recording errors and subsequent investigation through root cause analysis/Significant Events Analysis and sharing lessons learned with the wider team.

o Maintain chronic disease registers ensuring accurate data quality and implement strategies to achieve targets.

Education and Training



o Following identified training needs, participate in training, facilitation and clinical supervision of other practice nurses.

o Provide mentorship and guidance for junior staff.

o Participate in partnership programmes, raising awareness, detection and prevention of chronic diseases including the design and implementation of a recall system.

o Maintain a high level of expertise in specific chronic disease management, specifically Diabetes, CHD, COPD & Asthma and seek to develop new and improved skills.

o Advise members of the public on the concepts of health promotion, education, prevention and protection

o Seek out new knowledge of nursing and health by reading, enquiring and partaking in internal and external continuous education programmes.

o Take responsibility for personal development and education including in-house training.

o Actively participate in multidisciplinary education and training.

o Monitor and maintain consistency of professional standards and take appropriate action if standards are not met.

o Maintain professional development by attending relevant courses and study days to keep abreast of current knowledge and developments.

o Abide by the legal requirements and statutory rules relating to practice and maintain professional registration (revalidation) with the NMC.

o Participate in an annual appraisal.

COMMUNICATIONS AND WORKING RELATIONSHIPS



o The post holder will be expected to develop good working relationships with the following:

o GP Partners, fellow nurses and Salaried GP's, Practice Manager, Dispensary & admin teams.

o Other nursing disciplines within the practice team

o Statutory and voluntary agencies.

o Nursing Teams within the Towy/Teifi Collaborative

o The general public, patients, relatives and carers.

RISK MANAGEMENT



It is a standard element of the role and responsibility of all staff of Llanfair Surgery that they take a proactive role in the management of risk. This entails the risk assessment of all situations, taking appropriate actions and reporting of all incidents, near misses and hazards.

RECORDS MANAGEMENT



As an employee of Llanfair Surgery, you have a legal duty of care for all records that you gather, create or use as part of your work within the Organisation, (including patient health, financial, personal and administrative), whether paper based or electronic.
