Job Description

We are a high QOF achieving, well organised PMS training practice looking to expand our friendly clinical team. We are seeking to appoint an enthusiastic and committed Practice Nurse for 6 sessions over 3 days per week who wants to develop their experience and help maintain our current high standards of care.

Hours and salary are negotiable based on experience. Willing to invest well in a high-quality candidate.

EMIS Web & Docman10 clinical systems and remote access available

JOB DESCRIPTION

JOB TITLE:- Practice Nurse

QUALIFICATION:- Minimum Registered General Nurse (R. G. N)

GRADE:- NHS Scale F , G or above

HOURS OF WORK:- Negotiable

REPORTING TO :- Managing Partner

ACCOUNTABLE TO:- Partners

JOB SUMMARY:-

To provide nursing services to patients of the practice, including treatment in participation with the GP or independently, screening and advice with referral to the GP, and community resources as necessary and nursing management of patients with chronic diseases.

PRINCIPLE RESPONSIBILITIES:-



1. To provide nursing care. Including childhood immunisations, travel

Immunisations, wound care, ear syringing and other treatments as

directed by the GP.

2. To provide health assessment and screening to the practice population

with referral to the GP as necessary, for example 75 + assessment, new Patients, screening and cervical cytology.

3. To manage and follow up the patients with Asthma, COPD, Diabetes,

CHD and Hypertension and liaise with the GP in need.

4. To Perform Spirometry and ECG..

5. To co-ordinate health promotion clinics according to established protocols

with referral to the GP as necessary.

6. To act as liaison for referrals to community resources such as social

Workers, district nurses and community physio and occupational therapists.

7. To maintain accurate records of duties carried out.

8. To co-ordinate and facilitate requests for prescriptions.

9. To maintain the treatment room in an orderly and tidy manner ensuring

That apparatus and utensils are appropriately sterilised or cleaned.

10. To ensure that adequate items of treatment room stock and vaccines are

Maintained.

11. To participate in the administration, professional and educational

responsibilities of the practice nurse.

12. To carry out periodical audit on the patients with chronic diseases.

13. Some home visits for the housebound patients if necessary..

14. To undertake any other duties as may be determined from time to time

commensurate with the range of duties described.

15. To train and teach Health Care Assistant and help when needed.

This job description will be subject to periodic review and amendment as

Necessary in conjunction with the postholder.

SPECIAL CONSIDERATIONS:-

- Must have experience in EMIS WEB.

- The postholder must maintain a valid registration with the UKCC

- The postholder should have current membership of a professional

nursing organisation.

- The postholder should maintain professional development by attending

Courses and study days as agreed with the GP.

- The postholder should attend the nurse's forum as and when required.

Any other assignment given by the management to assist the needs of the practice as and when required.



Job Types: Part-time, Permanent

Pay: Up to 35,000.00 per year
