As a General Practice Nurse, you will provide high-quality nursing care, health assessments, and treatments to patients, working closely with GPs and other healthcare professionals. Your role will involve patient education, chronic disease management, and participation in health promotion activities, all while adhering to the NMC Code of Conduct and the partnerships policies
Main duties of the job
Conduct patient assessments including history taking physical examinations and vital signs monitoring relevant to role
Administer treatments and procedures such as wound care ECGs Doppler assessments blood pressure checks and injections
Provide immunisations including child and travel vaccinations and perform cervical screening
Provide contraceptive care and advice
Support the management of patients with chronic conditions such as diabetes asthma COPD and cardiovascular disease
Monitor and review treatment plans ensuring they align with best practices and patient needs.
Collaborate with GPs and other healthcare professionals to optimise patient care
Promote healthy lifestyles and offer guidance on diet exercise and smoking cessation
Maintain accurate and detailed patient records, ensuring all treatments and consultations are documented in the practices computerised system
Monitor and manage medical supplies, including the ordering of vaccines drugs and laboratory materials
Participate in practice meetings, contributing to discussions on patient care practice development and policy updates
Work as part of a multidisciplinary team, supporting colleagues and sharing expertise to improve patient outcomes
Maintain a Personal Learning Plan to ensure compliance with revalidation requirements and NMC standards
