The Lea Valley Health Primary Care Network delivers high quality, patient-centred integrated care services to improve outcomes for our local population, bringing together local health and care organisations (including social care, the voluntary sector and patient groups) to work collaboratively as part of the NHS Long Term Plan.

The post holder will be a Prescribing Pharmacist, acting within their professional boundaries, supporting and working alongside a team of professionals in General Practice. The post holder will work within a multi-disciplinary team in a patient facing role to take responsibility for areas of chronic disease management and medication reviews to proactively manage patients with complex pharmacy.

This role will seek to implement a more standardised approach to medicines management within the PCN member practices. The post holder will ensure the PCN integrates with community and hospital Pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload in Primary Care.Please DO NOT APPLY if you are a Pharmacist without Independent Prescriber Qualification or unable to see patients with minor illness.
