Job Description


As a Prevention and Engagement Worker, you will be working in close partnership with other health care professionals, partner agencies, service commissioners, and internal clinical/mental health teams to ensure that our clients receive prompt advice and treatment.
Main duties of the job
A key element of the role will be to assist in the delivery of an integrated service by completing assessments, recovery plans, referrals, support, care coordination and appropriate interventions (e.g. psychosocial, pharmacotherapy, harm reduction). You'll also be able to confidently and expertly assess WithYou clients, carrying out triage and comprehensive assessment, as required and making appropriate onward referrals to specialist agencies.
