Duties

Support patients with activities of daily living, including personal hygiene, mobility assistance, and nutritional support. Monitor patient progress and adjust care strategies as necessary, maintaining detailed documentation. Supervise and mentor junior staff or care assistants, fostering a team-oriented environment with strong leadership qualities. Collaborate with multidisciplinary teams to coordinate ongoing care and manage complex cases effectively. Ensure compliance with all health and safety regulations, maintaining a clean and organised work environment. Educate patients and their families on health management strategies to promote independence and well-being.

Experience

Proven experience working within assisted living facilities, nursing homes or home care environments. Demonstrable expertise in senior care, dementia care, Alzheimer's care, and medication administration. Previous management or supervising experience is highly desirable, showcasing leadership capabilities. Familiarity with developing and implementing personalised care plans tailored to individual needs. Strong understanding of healthcare regulations related to home health nursing practice. Excellent organisational skills with the ability to prioritise tasks effectively while maintaining compassionate patient interactions.
