Role Introduction
An exciting opportunity has arisen for enthusiastic and motivated Staff Nurses to join our developing Neighbourhood Response Team at Leeds Community Healthcare. This is a fantastic time to become part of a Response Service that is growing, evolving, and shaping new ways of delivering urgent community care across the city. The Neighbourhood Response Team is a core component of the NHS community services commitment to the NHS 10-year plan and development of Neighbourhood Health.
Our Neighbourhood Response Team provides timely, person-centred nursing care within patients' homes, supporting early discharge from hospital and preventing unnecessary hospital admissions. We work to clear timeframes of 0-2 hours, 2-4 hours and 4-48 hours, ensuring patients receive the right care, at the right time, in the right place.
We have bases across Leeds and offer a variety of flexible shift patterns between 7am and 6pm, seven days a week.
