Job summary
Role Purpose
To fulfil the role and responsibilities of the Named Safeguarding Person (NSP) for both children and adults at Mansfield Medical Centre, ensuring the delivery of a high-quality safeguarding service to staff and patients.
Key Responsibilities
Work as an integral member of the safeguarding team to promote the safety and wellbeing of children and adults at risk.
Provide advice, guidance, and relevant safeguarding information to internal staff and external professionals.
Support colleagues in identifying safeguarding concerns and determining the most appropriate plan of action for each case.
Contribute to safeguarding case reviews by gathering and sharing relevant information.
Maintain up-to-date knowledge of safeguarding policies, procedures, and legislation.
Liaise effectively with a range of professionals and agencies to ensure coordinated responses to safeguarding issues.
Main Duties & Responsibilities
1. Safeguarding Case Management
Liaise with internal and external professionals to identify and collate relevant health information for multi-disciplinary risk assessments and Multi-Disciplinary Team Meetings (MDTs).
Under the supervision of the Safeguarding Lead GP, prepare accurate and timely case conference reports and safeguarding health reports in line with the Nottinghamshire Safeguarding Children's/Adults Board (NSCB) protocol.
Proactively review patient case files in relation to Levels 2-4 of the Coventry and Rugby Thresholds and Pathways procedure.
Raise safeguarding concerns promptly with the Safeguarding Lead or deputies.
Ensure patient records are maintained in line with Mansfield Medical Centre's record-keeping policies.
Obtain informed, written consent before sharing information with external professionals, unless exempt under safeguarding policy.
Respond to requests for health information promptly and professionally.
2. Multi-Disciplinary Team (MDT) Coordination
Plan MDT meetings, distribute agendas, and record accurate minutes.
Maintain safeguarding databases and ensure information is updated following MDT discussions.
Attend key meetings as required, including practice MDTs.
3. Staff Support & Training
Provide safeguarding guidance to Mansfield Medical Centre staff, with support from the Safeguarding Lead and deputies.
Contribute to the delivery of safeguarding training and the distribution of training updates.
4. Audit & Data Reporting
Contribute to safeguarding metrics data collection to reflect team activity accurately.
Participate in safeguarding audits, including a three-month in-house audit and quarterly claims
Share learning from bimonthly training sessions with relevant staff.
